Mensagem:  I am speaking to you now.
Do you understand?

I just spoke to you.
Do you understand?
Did you understand?

I spoke to you an hour ago.
Did you understand?

The meaning of "Have you understood?", at least to me, has a slightly different meaning.  It's more like "Compreendeu?" than "Entendeu?"  It has more to do with understanding instructions or explanations than it does with understanding speech or the written word. 
If I have explained something to you, I may ask "Did you understand?" or "Have you understood?"  I see no major difference in meaning. 

(Yes, I am aware that in Brazil very formal rules are taught about "did" and "have", but they are inconsistent with what I hear among native speakers.)
