Mensagem:  Is it already nine o' clock? (as it appears in the quiz)
You are expressing surprise that the time is nine o´clock.


but...if you change the structure slightly, you have two possibilities:
(1) Is it nine o' clock already? or (2) Is it nine o' clock yet?
(1) You are expressing surprise that the time is nine o´clock.
(2) You are eager and impatient for it to be nine o´clock, and you asking if that is the time.
.......................................................................................................
The last question has two possible answers. 
still = You are saying that the person did not go to bed.
already = You are saying that the person went to bed but awoke very early.
