Yep, similarly I have heard the Brazilian version of that "dickhead"/"cara de..." in the northeastern states. Said almost naturally.
Same situation, that is, said by people of the lowest ranks in society and by extension of humble origins (it happens that I interact with people of all walks of life...), in normal conversation and in the presence of a girl/woman it would be very offensive, tough. More so, said by a guy with some degree of education and literacy.
Anyway, as an aside I had a boss that would use bad words no matter what, wether in presence of a female or not, he simply couldn´t help himself! under pressure he would swear like an old sailor!
