A town is a small city, and a city is a large town. I have never heard of a specific difference between the two.
A third term is "village". This can be translated as vila, vilarejo, or even aldeia. In a town, sometimes you will hear the commercial called "the village". A village, therefore, is not necessarily a small town. Sometimes "the village" is merely the business area of a town.
