How different is the pronunciation of "Up and at'em!" from "Up and at them!". You mentioned that nobody uses the second form, but when I pronounce them aloud, they sound the same...

No, they shouldn't sound the same.  The pronunciation of "at'em" should not have the "th" sound heard in "them".

 

Upan-AT-em versus Upan-AT-them

 

Note that the stress is on the "at".

 

Try saying the expressions a few more times.
