I don't think we use "third wheel" as the old lady escorting the kids on their date.  It's when you feel superfluous....extra.  A bicycle needs only two wheels.  A bicycle with three wheels wouldn't work.  The third wheel is of no use.  That's the same feeling one has when they are not needed.

 

If you are with a guy and gal and they want to be alone then you're the third wheel.
