"Macho" is commonly used to mean "macho" or "machista". Obviously, the context is of great importance in determining which meaning the speaker wants to project.
When wanting to say "strong, manly, in superb physical shape, and man among men" without the negatives commonly associated with "machista" or "chauvinist" it's common to hear simply "He's so Tom!"
