A doll murderer could be someone who murders dolls or a doll who murders. 

 

Take the term "Boy Scout".  Is he a scout who is a boy, or someone who scouts for boys?  He could be both.  When I was a Boy Scout, we often made jokes about "Girl Scouts".  Sure, we scouted for girls.  Why not?

 

And how about soldier boy and boy soldier?  They mean the same thing despite the sequence of the words.  A boy soldier or a soldier boy is a young (boyish) soldier.
