"A name or a title in direct address is parenthetic."
I read this sentence many times, and it simply didn't make sense. I think now, however, that the writer was saying that "A name or a title in direct address is redundant, unnecessary." If you are speaking with Dr. Altamiro Vilhena, he surely knows his name and title. So, using his name or title is redundant, unnecessary.
