Jeff Dunham is a great ventriloquist.   One of his skits is with the dimwitted Peanut character.  The hand passing over head gesture, accompanied by something sounding like a very rapid "zzoooooooommm", is to indicate Peanut missed something in the conversation such as the punch line of a joke.

 

I have never seen that gesture used in any other context, which is not to say that it is not used...
