a bolt = a lightning bolt
the blue = the sky
like a bolt from the blue = something completely unexpected, unplanned

I was having a glass of iced tea with Thiago's girlfriend when, like bolt out of (from) the blue, she kissed me.  Honest!  I mean, I was just sitting there.  I'm going back tomorrow to see if she does it again.

We were talking about Costa Rica's win over the USA last night when, like a bolt out of (from) the blue, the boss said he was promoting me to the manager slot in Miami.  It was so unexpected!

(Note that "like a bolt out of the blue" is often shortened to "out of the blue".  It can refer to something negative or positive, and "from the blue" is often changed to "out of the blue".)
