'How ever did you come up with that idea!' (implying 'you must be crazy' to have dreamed that up)
There is a much less polite version of the structure too: 'how the "bleep" did you do that?!'
In those sentences, "How" is the question-word.
How did you do that? correct
How = question word
did = auxiliary
you = pronoun
do = verb
that = object pronoun
and then we add the part to make the sentence complete
The other sentence can be rethorical. But you can still use the structure "How ever ... ?" just to add intensity to the question making the structure also non-rethorical.
