She didn't use to smoke. (Ela não costumava fumar.)
I believe we are overlooking an implication, and that is "Ela não costumava fumar, mas fuma agora." At least to my ears, that is what the writer is implying.
If at a point of time in the past she smoked but she does not smoke now, I'd suggest "She didn't smoke." Period. For example: "She did not smoke when she was a student in Alaska." "She did not smoke when we went to Rome." There is no implication that she smoked earlier or later.
